Our office participates with several managed care
plans. If you are covered
by one of these plans, we will file a claim for you.
Co-payments, if required by your insurance, are due
at the time of service. If
you do not have your co-pay, you will be asked to reschedule.
For Private/Commercial Insurance plans, payment is
due at the time of service. As
a courtesy, we will file a claim for you, and have the benefits paid
directly to you.
No Insurance/Self Pay, payment is due at the time
of service.
If we are not a participating provider for you
plan, we will treat your plan as we do private plans.
